What is Free Flap Surgery? Free flap surgery, also known as free tissue transfer, is one of the most complex and transformative procedures in modern reconstructive plastic surgery. It involves taking healthy tissue (which can include skin, fat, muscle, and even bone) from one part of the body—called the donor site—and transplanting it to another area that has suffered severe damage or tissue loss. Unlike a traditional skin graft, a free flap includes its own blood supply. Using high-powered microscopes, surgeons meticulously reconnect the tiny arteries and veins of the transplanted tissue to the blood vessels at the recipient site. This ensures the newly placed tissue survives and heals naturally. When is this Procedure Used? The Recovery Journey Recovery from a free flap procedure requires patience. Patients typically spend several days in an Intensive Care Unit (ICU) so that nurses can closely monitor the blood flow to the new flap using specialized Doppler devices. While the donor site usually heals like a standard surgical wound, the recipient site needs strict immobilization and care. Full integration of the tissue and return to normal activities can take anywhere from 6 to 12 weeks, accompanied by tailored physical therapy.
